Medical & Prescription Drugs
The cost of quality health coverage has increased significantly over the past few years. At the same time, we need health care that protects our physical health as much as health care the protects our financial well-being. That is why Pathway believes it is important to invest in quality plans that are cost effective, easy to use and valuable to you. We provide the following options for medical and prescription drug coverage:
- Humana Premier HMO
- No deductible, an out-of-pocket maximum of $1,500 for individuals and $3,000 for families, 100% coinsurance, $20 physician visit copay, 100% hospitalization coverage, $20 preventative care copay, $75 emergency room copay, and a retail and mail order prescription drug benefit.
- Humana HMO Select
- No deductible, an out-of-pocket maximum of $1,500 for individuals and $3,000 for families, 70% coinsurance, $30 or $40 physician visit copay, 70% hospitalization coverage, $30 or $40 preventative care copay, $150 emergency room copay, and a retail and mail order prescription drug benefit.
- Humana In-Network POS
- $1,500 deductible for individuals and $3,000 deductible for families, an out-of-pocket maximum of $3,000 for individuals and $7,000 for families, 80% coinsurance, $20 or $35 physician visit copay, 100% hospitalization coverage after the deductible and after $100 per day for the first three days, $20 or $35 preventative care copay, emergency room coverage of 100% after deductible plus $75 copay, and a retail and mail order prescription drug benefit.
- Humana Out-of-Network POS
- $3,000 deductible for individuals and $6,000 deductible for families, an out-of-pocket maximum of $7,000 for individuals and $14,000 for families, 60% coinsurance, physician visits at 70% after deductible, hospitalization coverage of 70% after deductible, preventative care of 70% after deductible, emergency room coverage of 100% after deductible plus $75 copay, and a retail prescription drug benefit.
Dental
Taking care of your teeth is as important as taking care of the rest of your body. That’s why Pathway offers dental insurance that covers routine check-ups and additional services needed for your health. We provide the following options for your dental coverage:
- Guardian Dental HMO
- No deductible, $5 office visit copay, preventative services limited to copays, basic services limited to copays, major services limited to copays, no annual maximum, and $1,000 savings in orthodontics.
- Guardian Dental In-Network PPO
- $50 deductible for individuals and $150 for families, 100% coverage of preventative services, 80% coverage of basic services, 50% coverage of major services, and an annual maximum of $1,000.
- Guardian Dental Out-of-Network PPO
- $50 deductible for individuals and $150 for families, 100% coverage of preventative services, 80% coverage of basic services, 50% coverage of major services, and an annual maximum of $1,000.
Vision
Eye care not only helps you see better, but routine check-ups can detect early signs of many different illnesses inclusing diabetes. Pathway offers vision benefits through Guardian’s Vision Service Provider (VSP) network. Benefits include routine vision exams, lenses, frames and contact lenses.
Group Life and Accidental Death and Dismemberment
Pathway provides life insurance through Humana to all full-time employees at no cost to the employee. Each full-time employee receives up to $15,000 group life and accidental death and dismemberment (AD&D) insurance.
Voluntary Life
Pathway employees who want to supplement their group life insurance benefits may purchase additional coverage for themselves and/or their dependents through Sun Life Financial. The cost of this benefit is deducted directly from employees’ paychecks.
Disability Income
Pathway offers full-time employees short- and long-term disability income benefits in the event of a disability from a non-work-related injury or sickness.
Flexible Spending Account
Pathway offers employees the opportunity to pay for out-of-pocket medical, dental, vision and dependent care expenses with pre-tax dollars through a Flexible Spending Account (FSA). Employees who participate in the FSA can save approximately 25% of each dollar spent on these expenses.
